When Moodle/PHP debugging is turned on, the integration is broken

      Description

      When Moodle debugging is turned on, then there are quite a few undefined variable statements that interfere with the javascript directory structure. Also it seems that the lib.xml.dom library that we have included collides with another library that has been added in Moodle 1.9.x

        Changed the constants that were colliding with the other moodle libraries.

        Fixed the undefined variables so even with debugging turned on, there's no more error messages that will prevent the LAMS-Moodle module to add sequences.
